This message, repeated frequently during medical appointments, may have even become a constant presence in your mind.

For some people living with type 1 diabetes (T1D), this vigilance can eventually take up so much space that it becomes a constant source of pressure. Every high blood sugar reading can then feel like a failure. Every deviation, a mistake. Some people may consequently feel tempted to aim for the lowest blood sugar levels possible.

For some individuals, this drive can become rigid, at the expense of their health. This phenomenon, known as hyperglycemia aversiveness or fear of hyperglycemia, is still poorly documented in the scientific literature, but is nevertheless very real in the daily lives of many people managing T1D.

How Can You Recognize When This Fear Becomes All-Consuming?

Research shows that fear of hyperglycemia is often fueled by a combination of fear of complications and very high personal standards. People affected may want to maintain very tight control over their blood sugar levels and experience frustration or guilt when they are unable to do so.

They are often driven by intense self-criticism: “I should be doing better,” or “I wasn’t strict enough.”

An all-consuming fear of hyperglycemia may manifest through certain behaviours or thoughts, for example:

administering insulin doses that are too high;

under-treating hypoglycemia out of fear that blood sugar levels will rise;

deliberately ignoring symptoms of low blood sugar;

viewing hypoglycemia as an “absolutely necessary evil” to maintain good blood sugar levels;

experiencing constant mental exhaustion related to diabetes.

If you recognize yourself in some of these behaviours, know that you are not alone and that this is not a question of willpower.

A Fragile and Sometimes Risky Balance

At first glance, keeping blood sugar levels low may seem reassuring, or even “safer.” However, prioritizing levels that are too low exposes people to very real risks.

Data show that people with a strong aversion to hyperglycemia do indeed spend more time within their target blood sugar range, but also spend more time in hypoglycemia, which is not without consequences.

Over time, ignoring the early signs of low blood sugar or treating them inadequately can cause the body to gradually stop sending its usual warning signals (e.g., trembling, sweating, or hunger). This is known as reduced awareness or impaired awareness of hypoglycemia.

When these warning signals disappear, hypoglycemia can progress silently, without clear early signs, and may only be detected once more severe symptoms appear, such as:

confusion;

disorientation;

or even loss of consciousness.

These situations can become particularly dangerous in everyday life and compromise safety and the ability to function—for example, while driving, during an exam at school, or while giving a presentation at work.

Occasional High Blood sugar Readings Do Not Define Diabetes Management

When fear of hyperglycemia takes up a lot of space, it can be helpful to remember one essential thing: diabetes complications do not develop because of a few hours or a few days of high blood sugar levels.

They generally develop after years of repeated and persistent high blood sugar levels. In other words, a higher reading after a meal, a more difficult day, or a period of stress does not erase all the effort put into diabetes management on a daily basis.

Blood sugar levels are influenced by many factors: hormones, stress, sleep, physical activity, digestion, illness, the menstrual cycle, and more. They cannot be completely controlled and are not solely a matter of willpower or discipline.

It is also useful to know that even people without diabetes experience rises in blood sugar after meals, often above 8.0 mmol/L. These fluctuations are normal. In fact, they may spend a small portion of the day—approximately 2% to 5%—in hyperglycemia without this being problematic.

Making Room for Safety… and Self-Compassion

Fear of hyperglycemia is understandable and can be useful… up to a point. Managing diabetes is not only about numbers; it is also about maintaining balance in life.

Adopting a more compassionate approach toward yourself does not mean giving up. It means recognizing that blood sugar levels cannot be completely controlled, and that setting realistic rather than perfect goals can help reduce pressure and prevent the cycle of frustration and self-criticism, without neglecting your health.

As part of this search for balance, preventing hypoglycemia should remain a priority. This includes:

preventing and promptly treating low blood sugar;

not minimizing or ignoring symptoms;

accepting the need to take the carbohydrates necessary to raise low blood sugar, even if this temporarily results in a higher blood sugar level.

It can be helpful to remember that safe diabetes management is about avoiding extremes, not achieving perfection.

Talking About It Is an Important First Step

Wanting to manage your T1D well is a strength. But when this desire is accompanied by excessive pressure, harsh self-criticism, or a fear that influences your daily decisions, it can become counterproductive and exhausting.

If fear of hyperglycemia is taking up a lot of space in your life, it may be helpful to discuss it with your healthcare team.

Finding a balance between rigour and flexibility, and between vigilance and self-compassion, is essential to protect both your physical and mental health.

To Learn More

Log in and explore our courses on:

Diabetes-related mental load, burnout, and distress

Hypoglycemia
